The Community Health Choices (CHC) Waiver Program offers a free transportation service to support older adults who may need assistance getting to medical appointments, grocery stores, and other essential destinations. Through this program, older adults can receive curb-to-curb transportation via shared van rides, allowing them to maintain their independence and access important services within the community. Riders are required to schedule their trips in advance, typically a few days ahead, to ensure availability and smooth coordination of transportation services. This gives older adults the opportunity to plan their trips accordingly and have peace of mind knowing they have reliable transportation arranged through the CHC Waiver Program. Thanks to generous donations, the CHC Waiver Program can provide this valuable transportation service at no cost to older adults enrolled in the program. By offering door-to-door assistance through shared van rides, older adults can comfortably travel to their destinations and continue to engage in social activities and appointments that contribute to their overall well-being.
